Student-Athlete Physical Information
Cosumnes River College Athletes:
- All student athletes need to pass a physical examination screening before participating in a sports-related activity at Cosumnes River College
- Student athletes need to secure their own physicals through their primary care physician or through urgent care. Los Rios Community College does not pay for sports physicals
- Physical exam must be dated on or after June 1 to be valid for the upcoming school year
- A MD, DO, PA, or NP must sign the physical form and include the medical practice/clinic stamp (digital or physical) on the form for verification purposes. Physicals performed by a chiropractor will not be accepted by the athletic training staff
- Below is the link Los Rios Preparticipation Physical form be sure to:
- print out both pages
- fill out the first page completely, in blue or black ink
- Include any brief explanations of any “yes” answers in the provided spaces.
- Write your name and birthdate on the second page. This form is to be completed by your doctor
- Upload a copy of your physical onto you ATS account
Cosumnes River College has moved to fully to electronic record keeping, there is a link provided below with instructions on how to access the Athletic Trainer System (ATS) website.
All student athletes are responsible for completing all the steps on the ATS portal – including filling out forms – prior to the start of participation. Some forms require an electronic signature and if the athlete is under 18 years old, a parent or guardian must also sign the forms.
The instructions in the following links will walk you through the entire process. If by the first day the of your sport class, if you have not completed the necessary steps, you will not participate until one of the Athletic Trainers informs your coach of your clearance.
